Before your first session you will be emailed a Client Intake Form to complete. Please print and fill out the form and bring it to your first appointment.
To provide proper therapy at every subsequent session it is the client’s responsibility to always inform the therapist of new and existing health conditions.
Appointments will be confirmed one to two days before the scheduled massage.
Our time is valuable and your appointment time is reserved solely for you. If you need to cancel or reschedule, do so within 24 hours of your appointment time or send notification that a friend or relative will come in your place. Otherwise, you will be charged for the session.
Gift Certificates, unless otherwise stated, must be used in full during one visit. For example, a 90 Minute gift certificate cannot be used as (3) 30 Minute sessions or a combination of (1) 30 Minute and (1) 60 Minute session. However, the value of the certificate can be applied to a costlier service. For example, a Gift Certificate with a face value of $100 can be applied to a Lomilomi Hawaiian Massage service leaving a balance due of $55.
Gift Certificate sessions canceled or rescheduled after the 24-hour grace period will incur a $50 late fee or forfeiture of 50% of the Gift Certificate value, whichever is less. For example, if your appointment is set for 2:00 pm on Friday then, to avoid fees or loss of Gift Certificate value, you must notify me by email, text or phone No Later than 2:00 pm Thursday of any schedule changes.
NO SHOW Gift Certificate appointments are considered redeemed and cannot be rescheduled or redeemed at a later date. A NO SHOW occurs when an appointment is missed without any prior notification being made. To maintain the value of your GC, please reschedule 24 hours in advance of your appointment.
Expired Gift Certificates will be honored at 50% of face value for up to one year after expiration. One year after the date of expiration, expired Gift Certificates will be honored at 20% of face value.
Please arrive for your session a few minutes prior than your scheduled appointment to allow time to discuss any new changes. New clients should expect up to 15 minutes of the initial appointment to be spent discussing any health issues with the therapist.
If you are late for your massage appointment you will be provided as much time as available within the scheduled time slot. You will, however, still be charged the full amount for your scheduled session.
Payment is due upon receipt of services. Cash, checks, credit and debit cards are accepted. Please make checks payable to Naomi Baker.
Sexual misconduct, in any form, is unacceptable. By scheduling a massage session, you understand that any illicit or sexually aggressive remarks, advances or gestures will result in the immediate termination of the session. If a session is terminated by me because of sexual misconduct, you agree to full payment of the scheduled appointment.
